We provide our care services 24 hours a day, 7 days a week, 365 days a year. We have a one-call solution program that allows us to take care of the specific needs of all age groups and patient needs. We work closely with discharge planners to make sure your transfer from a facility to your home is seamless. Within 48 hours of discharge from the facility we will visit you to create a personalized care plan that will include goals, training and education to ensure the client receives the desired outcome from recovery. Our plan includes a follow-up with their physician within a week after discharge to ensure that all medications are being used properly.
